Certification and Cedentials
Must have an active Florida Dental License, in good standing
Must be FULLY ENROLLED as a Florida Medicaid Provider
Credentialed with DentaQuest Dental Insurance
Credentialed with Liberty Dental Insurance
Education
DDS or DMD degree
