The Dementia Specialist will be responsible for, but not limited to:

Position Summary

CareOne Harmony Village is seeking an experienced and compassionate Dementia Specialist to lead clinical excellence, staff education, and resident-centered programming for individuals living with memory impairment. This leadership role collaborates closely with interdisciplinary teams to ensure best-in-class dementia care, optimal resident placement, and ongoing staff development.

Key Responsibilities Clinical Leadership & Program Management
  • Ensure timely, compliant documentation in accordance with federal/state regulations and facility policies
  • Serve as a subject matter expert and role model in dementia care best practices
  • Participate in interdisciplinary team meetings and care planning
  • Conduct cognitive assessments and reassessments (at least every 6 months or as needed)
  • Collaborate with Director of Wellness (DOW) to ensure appropriate resident placement and care levels
  • Communicate reassessment outcomes with families and guide care transitions
  • Maintain strong partnerships with healthcare providers and external vendors
  • Promote a safe, respectful, and supportive environment for residents and staff
Education & Staff Development
  • Design and implement monthly dementia training programs for all departments
  • Deliver onboarding and ongoing education to ensure staff competency in dementia care
  • Assess staff learning needs through observation and performance feedback
  • Provide 1:1 coaching and mentoring for staff requiring additional support
  • Educate dietary and other departments to enhance resident experience (e.g., dining engagement)
  • Lead family education sessions on dementia care and progression
Resident Services & Care Support
  • Support care partners in managing behavioral and cognitive challenges
  • Participate in pre-admission and ongoing assessments to ensure appropriate placement
  • Assist in new resident onboarding and family transitions
  • Monitor care plan compliance and collaborate with departments for updates
  • Foster a warm, structured, and calming environment throughout the community
  • Work collaboratively with private aides to ensure alignment with care standards
Marketing & Outreach
  • Partner with marketing team on community education initiatives and outreach events
  • Participate in pre-admission assessments to support occupancy goals
  • Contribute to community reputation as a leader in dementia care
Program Oversight & Collaboration
  • Review and align activity programming with resident cognitive levels
  • Meet regularly with department leaders to ensure staff readiness and training compliance
  • Participate in Manager-on-Duty rotation and leadership meetings
  • Support internal promotion and staff career development within Harmony Village
Qualifications Education
  • Bachelor’s Degree required
  • Master’s Degree preferred
Experience
  • Minimum 10 years of experience working with individuals with dementia or memory impairments
  • Prior management or leadership experience required
Licensure/Certification
  • Active RN, MSW, or OT license
  • Dementia certification required
