Deaf/Hard of Hearing Residental Care Coordinator

The Resident Care Coordinator is responsible for coordinating client care related to scheduling appointments, transportation, insurance, and communication with the care team. In addition, this position guides and assists individuals, that are deaf/hard of hearing and may have additional disabilities, to live and function as independently as possible in the community and participates in the development and implementation of individual program plans and coordination for the provision of specialized services. The Resident Care Coordinator must be fluent in American Sign Language.

REQUIRED: MUST BE FLUENT IN AMERICAN SIGN LANGUAGE (ASL)

DUTIES AND RESPONSIBILITIES:

  • Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors’ offices, health care providers and laboratories
  • Communicate and liaise with physicians, nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident
  • Document all physical, health care, dental, vision appointments
  • Prepare and complete all necessary documentation in accordance with program standards within required time frames as required
  • Perform Direct Support Services when needed
  • Travel between sites on a regular basis
  • Other duties as assigned

SUPERVISORY RESPONSIBILITIES:

This position has no supervisory responsibilities.

MINIMUM QUALIFICATIONS:

  • High School diploma or equivalent required, Bachelor’s degree in Human Service Field preferred
  • At least 2 years working in a residential setting required
  • Proficiency in Microsoft applications
  • American Sign Language is required

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

  • Regularly required to lift 10 lbs.
  • Must be able to lift a minimum of 15 lbs.
  • Must be able to pull a minimum of 10 lbs.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop.
  • Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
  • Required to regularly stand and walk.
  • Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing, and lifting.
  • Vision requirements include close vision, distance vision, and peripheral vision.
