Job Summary and Qualifications:

POSITION SUMMARY: Responsible for participating and leading pharmacy clinical programs in the area of Transplant services. This includes monitoring patient medication regimens and providing appropriate interventions, interacting with all medical and surgical specialties responsible for the care of transplant patients and collaborating with nursing staff to ensure optimal care of the transplant patient.

  • Provision of Care Coordinates and assists with the development, implementation and maintenance of clinical pharmacy programs to ensure clinical services meet the requirements and needs of the patient population and community.
  • Provide comprehensive pharmaceutical care to transplant recipients in a culturally competent manner to include evaluation, identification and solution of medication related problems for transplant recipients.
  • Prepares and assists with discharge planning for all transplant recipients. Evaluates transplant recipient medication regimen on a regular basis.
  • Provides clinical pharmacy support and executes clinical pharmacy programs for all patient areas.
  • Reviews medication orders and quality of technicians work to ensure appropriateness of drugs ordered, dosage, route and interactions. Adjusts medications dosage/schedule as appropriate for patient condition as per policy.
  • Ensures medications are safely dispensed to patients in accordance with age specific guidelines. Follows appropriate procedures for the sterile preparation of IV admixtures in compliance with USP 797 standards.
  • Participates in the evaluation and analysis of clinical data, medication use, drug and drug class monograph development in support of clinical toolkit development.
  •  Provides oversight of pharmacy distribution activities including order receipt, product storage, controlled substance process, dispensing and other distributional functions.

    What qualifications you will need:

    • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
    • Minimum of 3 years’ experience as Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience required; preferably within area of focus
    • Or completion of a PGY2 pharmacy residency or fellowship AND BCPS certification or other Board of Pharmacy Specialty (BPS) certification preferred
