Job Summary: The Critical Care/ED Pharmacist attends all hospital codes and priority alerts, facilitates emergency medication provision, and works to provide comprehensive pharmacy services for all patients of the hospital and ED.
Essential Job Functions & Accountabilities:
Attends Code Blues including traumatic arrests, provides necessary medications, and documents participation.
Attends Rapid Sequence Intubations in all areas of hospital and provides necessary medications.
Attends Code Strokes, reviews patient profile, makes recommendations for treatment, and mixes fibrinolytics if warranted.
Attends Code Sepsis, reviews patient profile, makes antibiotic recommendations, and mixes necessary medications.
Reviews and provides comprehensive pharmacy services for all STAT orders in the ED and ICU.
Monitors EPIC verification queue for all STAT orders and those requiring clinical intervention or iVents.
Performs drug utilization reviews, monitors for drug interactions, assists in maintaining integrity of formulary, participates in antibiotic stewardship program, provides pharmacokinetic dosing and monitoring services to the medical staff and evaluates potential adverse drug events.
Functions as a clinical resource for the hospital staff.
Ensures completion and accuracy of required documentation and record keeping.
