PositionSummary Qualifies patients for the Medication Access Program while working in collaboration with county agencies, case managers, pharmacies, hospitals, and doctors offices. Develops a teaching/learning plan for patient and patients family and provides education on the safe and correct use of medications and importance of their medication regime. Implements departmental policies and reassess policies for efficacy. Reports and evaluates effectiveness in terms of HCMAPs mission statement to Board and Steering Committee. Collaborates with pharmacies, case managers, and social service agencies in case findings and referrals to the Medication Access program. Primary Position Responsibilities Measures and tracks clinical and economic outcomes of program. Oversees HCMAP program, staff, and volunteer(s). Disseminates program information. Submits patient applications to pharmaceutical companies. Audits patient charts and processes. Grant reviews and preparation. Qualifications Minimum Education: Required: High School Diploma or Equivalent. NJ Licensed RN Associates Degree. Preferred: Bachelors Degree in Nursing. Certificate and/or Advanced Specialized or Technical Training in Diabetes Education. Masters Degree in Nursing Minimum Years of Experience (Amount, Type and Variation): Required: Five years nursing experience with 1 year patient/family education experience. Preferred: License, Registry or Certification: Required: New Jersey Licensed RN Preferred: Knowledge, Skills and/or Abilities: Required: Knowledge of pharmaceutical assistance programs, diabetes foundation, teaching/learning principals, behavior change and outcome measurements.
