Job Requirements
  • Education Requirements: Valid Registered Nurse (RN) license in Wisconsin
  • Certifications Requirements: BLS certification; Valid Driver's License; Auto Insurance
  • Experience Requirements: Minimum of 2 years of hospice care experience
  • Other Requirements: Comfortable working independently in private homes, skilled in symptom management for end-of-life care, proficient in electronic medical records (EPIC), and excellent communication skills with multidisciplinary teams

Job Responsibilities
  • Provide comprehensive hospice nursing care and symptom management to adult patients in private homes, SNFs, and assisted living facilities within the Sheboygan/Manitowoc area.
  • Conduct admissions visits, completing two patient assessments per shift, and educate patients and families on hospice expectations and medication management.
  • Collaborate closely with MDs, case managers, and liaisons to ensure personalized and compliant care plans are executed efficiently.
  • Document all patient interactions, interventions, and care plans accurately in EPIC EMR system, maintaining compliance with hospice and healthcare standards.
  • Monitor patient symptoms, provide interventions as needed for pain control and end-of-life comfort, and adjust care plans based on ongoing assessments.
