SUMMARY:

The Contract Liaison willeducate/monitor facilities and staffproviding information onthePACE model/policy andprocedure. Thisindividualis responsiblefor the establishment of a collaborative working relationship betweenPACESoutheastMichiganand our external stake holdingfacilities, i.e. nursing homes, assistedliving, group homes, dialysiscenters, and contracted providers. Thispositionisanintegral part ofour InterdisciplinaryTeamproviding usefulinformationthatwill complimentand improve our admissions process, accuratefinancial datareporting and ageneralincreasein our qualityofcare, service excellence andrelationship building withcontracted facilities. Thisindividual isresponsible foraproactive approachto educating external stakeholders in the PACE model and coordinating communication with internal staff.

SPECIFIC DUTIES AND FUNCTIONS:

  • Make a minimum of quarterly visits to our nursing homes and other facilities as needed.
  • Establish good working relationships with internal/external staff.
  • ProvideeducationtofacilitystaffrelativetoPACE,whoweareandwhat wedo.
  • Providecareconferenceinformationtointernalstafftoincreasecommunicationwithexternalstaffatnursinghomes.
  • Performageneralroundingonallparticipantstocheckontheirconcerns,reportingproblemsandconcernsbacktoIDT.
  • Serve as a communication resource to the RNCMs and Social Workers.
  • AttendIDTmeetingsperiodically/asneededateachsitetoretrieveorreportcurrentstatusofourfacilityparticipants.
  • ProvideMDSscorestofinanceinatimely fashion.
  • Ensure RNCM's have access to medical records in facilities.
  • Perform an annual inspection of contracted facilities.
  • Maintain a written record of facility inspections.
  • Provide education and annual reviews with contracted specialists/providers.
  • Escalate facility concerns to the Associate Medical Director or Quality Director.
  • Document all facility concerns and provide in writing to Quality Director.
  • Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Must be a Licensed Practical Nurse with current Michigan licensure.
  • Experience working in a Nursing Home .
  • MDSexperiencepreferred.
  • Good relationship building and customer service skills.
  • Have working knowledge of Utilization Review and Discharge Planning processes.
  • Have working knowledge of Resource Utilization Groups (RUGS).
  • Must be willing to travel between contracted facilities with reliable transportation.
  • Willing to sometimes work flexible hours.
  • Possess excellent organizational and prioritization skills.
  • Performs related duties as required or requested.
  • Creative, detailed-oriented and organized.
  • Excellent written and verbal communication skills with specific ability to maintain accurate records.
  • Stronganalytical skillsandattentiontodetail.Workingknowledge of computer softwareforword processing,formatdevelopment and spreadsheet management.
  • Ability to work with diverse populations/groups/teams.

WORKING CONDITIONS:

  • Prolonged periods sitting at a desk and working on a computer
  • Regular travel to PACE SEMI centers, community sites, events, etc.
  • Ability to lift up to 15 pounds as needed
