Job Requirements
  • Education Requirements: Registered Nurse (RN) license in the state of Wisconsin or compact state
  • Certifications Requirements:
    • BLS (Basic Life Support)
    • Valid Driver’s License
    • Auto Insurance
  • Experience Requirements:
    • Minimum of 2 years of experience in hospice or end-of-life care
    • Experience supporting home health, ALF, SNF, and hospital GIP settings
    • Familiarity with Epic and Rover EMR systems
  • Other Requirements:
    • Ability to support within the community at patient homes
    • Strong teamwork and communication skills
    • Must be detail-oriented and capable of working independently

Job Responsibilities
  • Provide comprehensive hospice nursing care to end-of-life patients in their homes, hospitals, and community settings
  • Assess patient conditions regularly, developing and updating individualized care plans
  • Collaborate with interdisciplinary team members to ensure seamless patient support and care coordination
  • Educate patients and families on disease progression, symptom management, and palliative care options
  • Document patient progress and interventions accurately within EMR systems such as Epic and Rover
