Candidates must be a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Social Worker (LSW) with a minimum of one year prior experience in-home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology, or other substitutes as approved by the ODA.
The Area Agency on Aging is seeking a dedicated Comprehensive Assessor to support individuals and caregivers in accessing long-term care services and community-based resources. This role is responsible for conducting in-depth, multi-dimensional assessments to evaluate physical, functional, social, and environmental needs, determine eligibility for Medicaid and waiver programs, and develop person-centered care plans.
The Comprehensive Assessor serves as a key link between consumers and essential services, providing education on long-term care options, facilitating referrals, and coordinating with internal teams and community partners to ensure timely and appropriate service delivery. This position requires strong clinical judgment, communication skills, and the ability to manage documentation and compliance requirements in accordance with state and agency guidelines.
The Area Agency on Aging, PSA 2 helps adults of west central Ohio remain in their homes with independence and dignity. We are an independent, private, nonprofit corporation that plans and funds services for older persons in Champaign, Clark, Darke, Greene, Logan, Miami, Montgomery, Preble, and Shelby counties. Through partnerships with over 250 community organizations, services are provided to older adults to help them remain independent. The Area Agency on Aging improves the quality of life for older adults in its nine-county community by advocating for individuals, developing and supporting a strong network of services, and providing resources for older adults.
