Summary:

Responsible for psychosocial support on selected patient populations and collaborates with the unit/ service CM/SW team for discharge planning and contributing to the patient's treatment plan through and understanding of social, psychological and/or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

Responsibilities:

Coordinates and collaborates with the unit/ service CM/SW staff around the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting around follow-up community behavioral health services. Collaborates with the behavioral health interdisciplinary health care team to ensure continuity of care and support services to patients and families. This includes a AP and psychiatrist assigned to the MEND program. Identifies and attends to the psychological needs of patients and families. Provides counseling to patients, families and groups. Responsible for documentation in the medical record, statistics and reports as required. Maintains relationships with community agencies and keeps up-to-date on resources. Uses effective interpersonal skills in all interactions. Enhances own professional development and maintains formal and informal continuing education for self-improvement.

Credentials:

  • Licensed Social Worker (Preferred)
  • MSW or LSW (Preferred)

Education or Equivalent Experience:

  • Master of Arts or Science in Social Work (Required)
  • 3+ years Behavioral health experience in the field, health care setting experience helpful, direct practice experience helpful.
