We're hiring a Community Support Team Leader (CST Lead) to oversee clinical and administrative operations for the Community Support Team (CST) in North Carolina. In this role, you'll provide supervision, coordination, and direct clinical support to the team, ensuring the delivery of high-quality services to consumers enrolled in CST Services. You'll work collaboratively with team members to develop and implement Person-Centered Plans (PCPs), monitor client progress, and make adjustments as needed to achieve positive outcomes. This role combines leadership with hands-on clinical practice, allowing you to make a direct impact on client care.
Provide clinical and administrative supervision to the CST team
Develop, implement, and monitor Person-Centered Plans (PCPs) for clients
Ensure services are aligned with the PCP and adjust interventions based on ongoing assessments
Evaluate the effectiveness of interventions and client outcomes
Collaborate with other service providers and multidisciplinary teams to deliver integrated care
Participate in team meetings, training, and development to enhance service delivery
Provide direct clinical services and support to clients as needed
Maintain accurate documentation and ensure compliance with agency standards and regulatory requirements
Master's degree in a human services field (e.g., Social Work, Psychology, Counseling)
At least one year of full-time, post-graduate experience with the MH/DD/SAS population
Full or provisional LCSW, LCMHC, or LMFT licensure in North Carolina
Experience in leadership or supervisory roles within behavioral health or social services
Knowledge of Person-Centered Planning and evidence-based practices
