Join the Robert Young Center as a Community Support Specialist! As a member of the Iowa Assertive Community Treatment (ACT) team, you'll provide trauma-informed, community-based case management and support to adults with severe mental illness. You'll coordinate treatment plans, provide crisis intervention, connect clients with community resources, and collaborate with an interdisciplinary team to help individuals achieve their recovery goals.
This position serves clients throughout the five Iowa CCBHC counties: Scott, Muscatine, Clinton, Jackson, and Cedar. Candidates who reside in Scott, Muscatine, or Clinton County are preferred, as office space is available in these locations and the majority of clients are served there.

Responsibilities:
- Provide case management and ongoing support for adults with severe mental illness in community-based settings.
- Complete diagnostic assessments, develop individualized treatment plans, and coordinate care throughout the recovery process.
- Deliver individual and group counseling, crisis intervention, and connect clients with community resources and support systems.
- Collaborate with an interdisciplinary team, including providers, nurses, social workers, and behavioral health professionals, to optimize client outcomes.
- Educate clients and families on mental health conditions, medications, treatment plans, and daily living skills.
- Maintain accurate, timely clinical documentation while ensuring compliance with HIPAA and all regulatory standards.
- Deliver compassionate, trauma-informed, person-centered care that promotes recovery, independence, and overall well-being.
