Join Our Team at Valley-Wide Health Systems, Inc.

Valley-Wide Health Systems is looking for a Community Support Specialist who is passionate about helping others and making a meaningful impact in the community. This role provides support, care coordination, and life skills assistance to help patients navigate challenges and improve overall well-being.

Key Responsibilities

  • Provide care coordination and life skills training for patients with significant mental illness individually and in group settings.
  • Manage caseload for Community Support Services including the Permanent Supportive Housing Program.
  • Develop, implement, and monitor individualized service plans.
  • Coordinate care in clinic settings, patient homes, and community environments.
  • Document patient interactions and maintain accurate clinical records.
  • Ensure timely documentation (within 48 hours) and meet productivity and compliance standards.
  • Collaborate with clinical teams to support patient care goals.
  • Attend team meetings, individual supervision, and group trainings as scheduled.
  • Demonstrate cultural competence and sensitivity in all interactions and documentation.
  • Provide transportation using a company vehicle to patients in behavioral health services as necessary.
  • Uphold HIPAA requirements and maintain strict confidentiality at all times.

Requirements

  • Bachelor's degree in Counseling, Psychology, Social Work or related field.
  • Candidates with non-related degrees may be considered with one year of relevant experience.

Preferred Qualifications

  • Valid Colorado driver's license and ability to travel as needed.
  • Ability to pass a Behavioral Health background investigation.
  • Strong written and verbal communication skills.
  • Proficient in Microsoft Office and general office equipment.
  • Demonstrates empathy, professionalism, and cultural sensitivity.
  • Excellent time management and organizational skills.
  • BLS Certification (provided upon hire).
  • Team-oriented with a collaborative mindset.

Location & Type of Job: This job is a full-time onsite job located in La Junta, Colorado.
