Overview:

To provide or arrange for the provision of services necessary to enable consumers to live, work and recreate in the community environment. The Community Support Specialist shall assist consumers with monitoring mental health symptoms, transportation, supportive relationships, and communication with other providers. Assisting also in attending appointments and obtaining medications, crisis intervention and development of a crisis plan and development and coordination of natural mental health support systems.

 

Hours:

Monday - Friday 8-4:30pm

Responsibilities:
  • Assess client needs and develop goals that support greater independence and successful community living.
  • Collaborate with clients and team members to identify creative alternatives to hospitalization, higher levels of care, homelessness, or incarceration.
  • Provide skill-building support, advocacy, and respectful care for clients with a wide range of abilities, needs, and mental health diagnoses.
  • Demonstrate sound judgment, teamwork, safe community-based service delivery, accountability, and commitment to meeting service and revenue expectations.
  • Complete timely, accurate documentation that meets funder, accreditation, prioritization, and monthly outcome tracking requirements.

 

Qualifications:

Education:

Bachelor’s Degree Required. Preferred in Human Services or related field.


Experience:

One year of Behavioral Health experience preferred. 


License(s)/Certification(s):

Valid driver’s license and insurance when driving any vehicle for work-related reasons.


Knowledge/Skills/Abilities:

Ability to speak, read and write fluent English. Ability to read and comprehend common terminology of the mental health field. Ability to enter documentation in an EHR and use Microsoft Office. Ability to effectively present information and respond to questions from clients, medical professionals, management, staff and the public.
