Position Summary  
The purpose of the Community Support Program Coordinator is to facilitate support and to identify resources for adults diagnosed with a mental illness. The goal of this position is to keep individuals living successfully in the community in ways that are client-centered, and assists with and/or provides services and activities to help achieve those outcomes. The Community Support Program Coordinator has a significant amount of contact with clients served and external persons or organizations such as other service providers.

Essential Job Functions  
Provide support and assistance to identify necessary resources (60%)
  • Assist with basic living skills, social, and coping skills training
  • Faciliate participation in recreational activities and groups (illness management and recovery, vocational, healthy living, etc)
  • Organize and facilitate weekly groups
  • Assist clients with clinical problem-solving, crisis prevention and crisis resolution
  • Help to identify needed community resources and make referrals internally and externally
  • Encourage client participation in a routine of productive daily activities, including but not limited to recreation activities, volunteerism, job and/or school
  • Coordinate with providers including client’s support team, family, psychiatrist, pharmacies, and case managers
  • Serve as a liaison for clients as needed

Support client vocational support (20%)
  • Encourage client to build network of peer support and overcome barriers to employment
  • Coordinate with employment service providers
  • Provide vocational support in classroom settings
  • Assist with resume development, interview prepation

Coordinate and complete all necessary paperwork and documentation (20%)
  • Track and document all necssary information for grant reporting
  • Coordinate completion of client documention (ie, benefits management, DB101 profile, SGA tracking and MA renewals)

Actual duties and percentages may vary slightly depending on jobsite location and assigned coverage

Qualifications
Education and Experience
Required
  1. Must meet Department of Human Services criteria for Mental Health Rehabilitation Worker defined by Minnesota Statutes 256B.0623 and Minnesota Statutes 245.462
  2. Valid driver’s license and insured vehicle
  3. Smart phone with the ability to interact via specific programs and apps

Preferred
  1. Bachelor’s degree in social work, psychology, public health or other human services related field, combined with at least one (1) year of experience working with adults who have been diagnosed with a serious and persistent mental illness
  2. Meet Department of Human Services criteria for Case Manager and/or Mental Health Practitioner
  3. Fluent in a non-English language or competent in the culture of an ethnic group to which 20 percent or more of the worker’s recipients belong

Knowledge Areas
  • Psychology – Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research messages; and the assessment and treatment of behavioral and affective disorders
  • Therapy and counseling – Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, career counseling and help
  • Sociology and anthropology – Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures and their history and origins
  • Customer service – Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
  • Education and training – Knowledge of the principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects
 
  • Computers and electronics – Knowledge of electronic equipment and computer hardware and software, including applications
  • Clerical – Knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, designing forms, and other office procedures and terminology
  • Language – Knowledge of the structure and content of language including the meaning and spelling of words, rules of composition, and grammar

Pay range for this position $27.02 - $29.27
