Job Summary

We are seeking a compassionate and community-focused Community Resource Coordinator to support underserved populations by helping members better manage their health and reduce inappropriate emergency room utilization. This role serves as a liaison between the community and the health plan, helping improve member outcomes, close care gaps, and increase overall member satisfaction.

Community Resource Coordinator Compensation

• The pay for this position is $25/hr
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

  Community Resource Coordinator Responsibilities

  ·         Conduct face-to-face and remote care management functions

  ·         Build and maintain relationships with community-based organizations

  ·         Serve as a liaison between health plan members and community resources

  ·         Educate members on appropriate healthcare utilization and preventive care

  ·         Assist members in navigating healthcare and social service systems

  ·         Support initiatives focused on reducing inappropriate ER utilization

  ·         Participate in quality and compliance case audits

  ·         Maintain accurate documentation and case records

  Community Resource Coordinator Requirements

  • High School Diploma or GED
  • 1–2 years of related experience
  • Strong communication and interpersonal skills
  • Ability to work independently in a remote environment
  • Experience working with underserved or vulnerable populations

  Preferred Qualifications

  • Community Health Worker (CHW) Certification
  • Experience in healthcare, social services, or care coordination environments
