This role serves as a vital liaison between the community and City Human Services, with a primary focus on supporting senior residents at Sunnyside Health Center. The individual in this position will provide essential resources, referrals, and information, while fostering strong connections between the senior program and Congregate services.
Responsibilities may include occasional visits to patients' homes to conduct wellness checks and ensure access to appropriate support services. The ideal candidate will demonstrate a high level of professionalism, compassion, and respect when working with elderly individuals and diverse community members.
Additional duties may be assigned as needed to support program operations and community engagement efforts.
KNOWLEDGE: Basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or a GED.
EXPERIENCE: Six months of experience are required.
