Summary

Home Care Agency is seeking a driven and relationship-focused Community Outreach Representative to promote and grow our presence within NYC and the greater surrounding area. This role is responsible for developing and maintaining strategic community partnerships that generate quality referrals for home care services. We are looking for candidates with home care marketing or community outreach experience, knowledge of MLTC and Medicaid programs, and the ability to build strong referral relationships within the downstate New York market. Bilingual candidates are strongly encouraged to apply. 

Essential Duties & Responsibilities: 

  • Build, maintain, and strengthen relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations. 

  • Develop and manage referral sources to drive census growth and increase community awareness of agency services. 

  • Promote agency services through community outreach efforts, presentations, networking events, support groups, health fairs, and community-based events. 

  • Conduct home visits, intake support, and personalized appointments as needed to assist with the enrollment process. 

  • Follow up promptly on referrals and leads received from families, caregivers, healthcare providers, and community partners. 

  • Maintain ongoing communication with referral sources to ensure strong partnerships and exceptional customer service. 

  • Submit daily activity and outreach reports to management. 

  • Represent the agency professionally within the community and identify opportunities for market growth. 

Qualifications & Skills:

  • Minimum of 1 year of sales, healthcare marketing, or community outreach experience required; home care industry experience strongly preferred. 

  • Knowledge of MLTC, Medicaid, and community-based healthcare services preferred. 

  • Strong existing relationships within the NYC/downstate healthcare and community network are highly desirable. 

  • Excellent communication, presentation, and interpersonal skills. 

  • Self-motivated, organized, and goal-oriented with the ability to work independently. 

  • Ability to coordinate and host community events and presentations. 

  • Must have reliable transportation and the ability to travel daily throughout the assigned territory. 

  • Bilingual in Spanish and English preferred. 

Work Environment:

  • Field-based / on-the-road position 

  • Full-time, 8-hour shift

Pay Range: The hiring range for this position is $55,000–$60,000 annually, plus commission. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
