Looking for a flexible LPN role with variety, independence, and meaningful community impact?

Cornerstone VNA is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in a unique, community-based role that blends in-home care with wellness clinics.

This position is ideal for an LPN looking for consistent part-time hours with flexibility, or those interested in expanding into a full-time dual role.

This is a dynamic position combining in-home clinical care, staff collaboration, and community wellness programming, with a strong emphasis on coordinating and providing foot care clinics, which are a required component of the role.

Key Responsibilities

Community Wellness & Clinics (Core Focus)

• Lead and coordinate foot care clinics in community settings such as senior centers and local organizations
• Plan and participate in wellness initiatives including vaccinations, screenings, and health education
• Build relationships within the community through outreach and preventive care

Non-Skilled Clinical Services & Home Visits (Under the supervision, direction, and collaboration of a Registered Nurse)

• Conduct home visits to assess patient needs and provide care in accordance with VA, CFI, state waiver, and self-pay program requirements
• Develop and support individualized care plans in collaboration with the RN
• Conduct home visits to assist with monitoring of patient needs and report observations with the RN
• Assist with scheduled home visits as assigned and directed by the supervising RN

Collaboration & Support

• Partner with clinical leadership and interdisciplinary teams
• Provide guidance and education to staff and community members
• Connect patients with appropriate services and resources

Schedule & Flexibility

• Structured 24-hour/week position

• Combination of:

• Scheduled clinic hours

• Flexible home visit scheduling

• Opportunity to expand into a full-time role through our Home Care departments

Work Environment

• Community-based role with travel throughout the service area
• Blend of clinic settings, home visits, and community locations
• Mileage reimbursed at the federal Mileage Reimbursement Rate

What Were Looking For

• Current New Hampshire LPN license
• Minimum of 2 years of nursing experience
• Comfortable working both independently and as part of a team
• Interest in community health, wellness, and preventive care
• Willingness to lead and perform foot care clinics as a regular part of the role
