Community Navigator - Outpatient Care Management

As a Community Navigator, you'll play a key role in supporting patients and families as they move through the healthcare system. You'll be part of an outpatient care management team dedicated to improving health outcomes through compassionate support, community outreach, and coordinated care.
What You'll Do
What You Bring
Education: High school diploma or equivalent required; Associate Degree preferred
Experience:
Clerical experience in a healthcare setting preferred
Familiarity with Microsoft Office, fax systems, and electronic medical records preferred
Skills & Abilities:
Strong communication and computer skills
Knowledge of community resources (preferred)
Basic understanding of medical terminology
Ability to prioritize tasks, stay organized, and manage multiple responsibilities
