the comfort of their homes. We are seeking a motivated, relationship-driven Community Liaison to
expand our referral network and strengthen partnerships throughout the community.
Job Responsibilities
• Build, develop, and maintain strong relationships with referral sources.
• Meet and exceed monthly patient admission goals.
• Coordinate with hospitals, inpatient rehabilitation facilities, skilled nursing facilities, physician
offices, and other healthcare providers to facilitate patient care and secure referrals.
• Conduct regular in-person visits to referral partners within the assigned territory.
• Represent Boundless Home Health professionally at networking events and community functions.
• Be available for phone calls and attend in-office meetings as required.
• Maintain accurate records of referral activities and provide regular updates to leadership.
Qualifications
• Minimum of 2 years of Home Health marketing experience required.
• Established book of business and existing referral relationships within the Home Health industry is
required.
• Excellent communication, networking, and relationship-building skills.
• Self-motivated with the ability to work independently.
• Valid driver's license and reliable transportation.
• Knowledge of the local healthcare market is strongly preferred.
