The Community Liaison represents St. Luke's University Health Network (SLUHN) Home Health and Hospice to build and enhance relationships with referral sources throughout the Network and Community; develops and plans educational programs, visits provider offices, nursing facilities, assisted and personal care communities to promote the Home Health and Hospice philosophy of SLUHN; participates in Community events to promote and educate the public at large about the SLUHN Home Health and Hospice services; and works closely with the Director of Clinical Referrals and Liaisons, Hospice Liaisons, Development and Marketing departments to coordinate these outreach efforts. JOB DUTIES AND RESPONSIBILITIES:Facilitates interagency communication and continuity of patient care by acting as the VNA Home Health/Hospice representative at the local health care facilities.Provides educational forums to hospital staff, medical community and community at large as requested.Directs all referrals to Intake Department in a timely manner.Acts as a resource to hospital staff and the community regarding Home Health/Hospice.Assists the Director of Referrals and Liaisons with activities as delegated, maintains schedule and sets up meetings with physician, physician offices, hospitals, case managers, nursing homes to market and educate on programs.Works collaboratively with Management, VNA professional staff and St. Luke’s Hospital & Health Network Marketing/Communications Manager assigned to the VNA of St. Luke’s to develop and implement a comprehensive marketing program for assigned services and programs.Increases growth in the Home Health/Hospice departments through direct visits and related educational activities.Maintains schedule to make presentations to promote programs to appropriate organizations and businesses.Supports the department’s special events and participation in those activities that promote Home Health/Hospice marketing and recruitment initiatives. In conjunction with the management team, plans, coordinates and organizes the contracted hospice providers’ education to meeting the demands of the department.Provides monthly statistical data needed for calls made, marketing activities and educational presentation within the reports.Creatively communicate St. Luke’s home health/hospice mission to all constituencies through personal interaction and public forum.Ability to establish rapport and trust with Hospice Management, employees and VNA and Network departments.Maintains confidentiality of all materials handled within the Network Entity as well as the proper release of information.Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.Demonstrates/Models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process and Safety.Demonstrates financial responsibility and accountability through the effective and efficient use of resources and activities to grow market share.Complies with Network and departmental policies regarding attendance and dress code. EDUCATION:Bachelor’s Degree or certification in a health-related field required OR Graduation from a School of Nursing with a current license as a Registered Nurse or Licensed Practical Nurse preferred. TRAINING AND EXPERIENCE:Two (2) years’ experience in Marketing, Sales or a related field preferred.
