Fourth Street Clinic's Street Medicine Team meets individuals experiencing homelessness where they live-on the streets, in encampments, and other outdoor locations across the Salt Lake Valley-to provide vital, lifesaving healthcare. Our program is the only one of its kind in Utah and among the few in the country, pioneering a care model that brings healthcare directly to the people who need it most.

Position Overview

The Street Medicine Community Health Worker (CHW) plays a critical role on the Street team, acting as a trusted liaison between people experiencing unsheltered homelessness and our integrated care services. The Street CHW helps break down cultural, linguistic, systemic, and emotional barriers to care by providing direct, on-the-ground outreach and connection to services. This role centers around meeting patients where they are-literally-through street outreach in encampments, alleyways, parks, and other non-traditional settings where people reside.

As part of a compassionate, trauma-informed team, the Street CHW collaborates closely with medical providers, behavioral health clinicians, case managers, and outreach partners to identify individuals in need of care, build trust over time, and support access to health care, social services, and long-term stabilization resources

QUALIFICATIONS & COMPENTENCIES

Required

  • High School diploma or equivalent.
  • Experience working with individuals experiencing homelessness, behavioral health challenges, or substance use.
  • Valid Utah driver's license and proof of insurability.
  • Strong interpersonal and communication skills with the ability to establish professional boundaries.
  • Commitment to trauma-informed care, harm reduction, and health equity.
  • Ability to work outdoors and in mobile, team-based settings.
  • Community Health Worker certification or willingness to obtain.
  • Demonstrated skills in de-escalation, problem-solving, and collaboration.

Preferred

  • Experience in community outreach, healthcare, public health, case management, or homeless services.
  • Familiarity with local homeless service systems and community resources.
  • Proficiency with spreadsheets, data tracking, and basic office software.
  • Bilingual or multilingual abilities.
