We are seeking a dedicated Community Health Worker (CHW) who provides patient centered support in the ambulatory care setting under indirect supervision. In this role, you will collaborate with Care Managers, Medical Providers, and the broader health care team to deliver high quality support aligned with established standards of care. You will also connect patients to community based programs that enhance their health, well being, and access to essential services through education, coordination, and resource navigation.

Responsibilities:
As a Community Health Worker, you will play a vital role in supporting patients, strengthening care coordination, and addressing social determinants of health. This position requires a compassionate, resourceful, and community‑focused individual who can build strong relationships, empower patients, and serve as a knowledgeable resource for both patients and the care team.
