Outreach and Engagement

  • CHW will conduct community visits, participate in community events, and build relationships to connect individuals with health and social services. They will conduct health screenings and provide referrals. They will drive and help maintain the Mobile Health Unit.

Health Education and Promotion

  • CHW will provide information on various health topics, including disease prevention, healthy lifestyles, and available resources by organizing support groups and educational workshops.

Navigating the Healthcare System

  • CHW will assist individuals in understanding and processing healthcare services, scheduling appointments, and following up via text, email, or phone call. They may also assist with applications for health insurance and other social services.
  • CHW will provide ongoing support to individuals with chronic conditions helping them manage their health and access necessary care. Playing a crucial role in improving health equity by bridging the gap between healthcare systems and communities, empowering individuals and families to lead healthier lives.
  • CHW will collect data on community health needs and relay this information to Mobile Health Unit Tucson Project Coordinator to inform program planning and service delivery.

  • Ability to communicate effectively with different groups/individuals.
