PositionSummary
The Community Health Worker provides culturally responsive, community-based services that address social drivers of health and support individuals in accessing health care, behavioral health services, and essential resources. This role focuses on care coordination, system navigation, health education, and advocacy to improve health outcomes and overall stability in alignment with Medi-Cal CHW benefit requirements.
Key Responsibilities
Participant Engagement and Assessment
- Conduct intakes, eligibility verification, and informed consent processes.
- Administer Social Drivers of Health (SDOH) screenings and needs assessments.
- Collaborate with participants to develop individualized action plans using SMART goals.
- Engage participants using a strengths-based, trauma-responsive, and culturally relevant approach.
Care Coordination and System Navigation
- Providehealth education andinformation tailored to participantneeds.
- Assist participants in accessing medical care, behavioral health services, benefits, housing resources, employment supports, transportation, and other community services.
- Coordinate referrals and follow up to ensure services are accessed and effective.
- Accompany participants to appointments when necessary to support engagement and continuity of care.
- Advocate on behalf of participants to reduce barriers to care and services.
Documentation,Billing,andCompliance
- Document all CHW services accurately and timely in the electronic health record.
- UtilizePIP(Problem,Intervention,Plan)formatforallprogressnotes.
- Ensure documentation meets Medi-Cal, managed care, and internal compliance standards.
- Track service delivery, referrals, and outcomes as required.
- Participate in documentation reviews, audits, and quality improvement activities.
