Immokalee Training Site - College of Medicine

Responsibilities

  • Provide community-based support, outreach, and care navigation for children, families, and pregnant or parenting individuals with mental health needs.
  • Coordinate patient support services, including screenings, psychoeducation, and linkage to care.
  • Deliver training and support to Community Health Workers and contribute to workforce development initiatives.
  • Engage in community outreach to promote early identification of concerns, reduce stigma, and connect individuals to health and social services.
  • Collaborate with interdisciplinary teams and support research and program activities.
  • Contribute to the education of healthcare trainees through community-informed perspectives and experiential learning opportunities.

Qualifications

  • Bachelor's degree in social work, public health, psychology, or a related field and two (2) years of relevant experience in community-based services, case management, behavioral health, or social services. (Note: higher education may substitute for experience at the equivalent rate.)
  • Valid FL driver's license or ability to obtain prior to hire. Ability to travel locally.

Preferred Qualifications

  • Experience working with children, families, or pregnant and parenting individuals.
  • Knowledge of community health systems, mental health services, and care navigation.
  • Experience in community outreach, health education, or Community Health Worker initiatives.
  • Bilingual (e.g., English/Spanish) strongly preferred.
  • Familiarity with trauma-informed care and responsive practices based on individual needs.

Anticipated Salary Range

$40,910 - $47,000, commensurate with experience and education, plus a robust benefits package (refer to FSU Total Rewards section).
