Position Summary: The Community Health Worker II plays a vital role in supporting individuals and families through proactive outreach, engagement and service coordination. This position focuses on delivering a broad range of supportive services, including life skills development, employment assistance, access to meals and groceries, mental and outpatient health service navigation, transportation support, case management and housing stability resources. This position requires strong communication skills, knowledge of community resources, and the ability to build trusting relationships with diverse populations.

Starting salary for this position is $19.75-$22.11 per hour DOE
Full range is $19.75-$31.13 per hour

Position Summary: This Community Health Worker II position will be focused on providing culturally appropriate health education and information, conducting outreach, building relationships with referral sources, coordinating care and helping clients navigate the health and social determinants of health systems.
