AEIC Case Management LLC is a trusted provider of mental health, wellness, and community-based support services across Texas. Our Bridgewell program specializes in providing high-quality Medicare support services—including care coordination, SDOH resource navigation, ongoing client engagement, and health behavior interventions—to improve overall well-being and reduce gaps in care for adults and older adults.


We are seeking highly qualified Community Health Workers (CHWs) who are passionate about supporting diverse communities, including seniors, adults with chronic conditions, and individuals facing social or medical barriers.


Job Summary

We are hiring Texas DSHS-certified Community Health Workers (CHWs) as 1099 Independent Contractors to provide resource navigation, care coordination, client outreach, and Medicare-aligned support services within the Bridgewell program.


CHWs will help clients access medical, behavioral health, housing, transportation, food, and wellness-related resources—while fostering empowerment, stability, and improved health outcomes.


Required Certification – No Exceptions

To qualify, applicants must already hold a valid Texas DSHS Community Health Worker (CHW) Certification.


Required:
  • Provide CHW Certification Number

  • Provide Expiration Date

  • Certification must not expire within two years of enrollment

  • Applications without CHW credentials will be automatically denied

Experience Requirements

Minimum of 1 year working as a CHW, case manager, or in a similar health or social services role — mandatory
Experience serving:


  • Older adults

  • Adults with chronic conditions

  • Low-income or vulnerable populations

  • Individuals facing barriers with transportation, food, housing, or medical access
Applications that do not meet the 1-year minimum will not be considered.


Skills & Competencies

  • Excellent interpersonal and communication skills

  • Ability to build rapport and maintain client trust

  • Strong understanding of community resources, SDOH needs, housing, and healthcare navigation

  • Strong organizational and documentation skills

  • Ability to work independently and manage your own schedule (contractor role)

  • Proficiency in Microsoft Office, Teams, and electronic record systems

  • HIPAA compliance and confidentiality awareness

  • Bilingual (English/Spanish) strongly preferred (incentive available)

Key Responsibilities (Bridgewell Program)

  • Conduct CHW-aligned assessments to identify social, medical, and resource needs for Medicare clients

  • Provide SDOH resource navigation (housing, food access, transportation, financial assistance, etc.)

  • Assist clients with obtaining medical, behavioral health, and community-based services

  • Coordinate care and follow-up support for clients with chronic illnesses or high-risk conditions

  • Encourage preventive care and adherence to care plans

  • Provide local education on healthy living, wellness, and navigating services

  • Document all client interactions, resource referrals, education, and outreach

  • Track client progress and assist with monthly monitoring requirements

  • Maintain accurate and timely documentation in the electronic health record

  • Communicate professionally with medical providers, clinics, and community partners

  • Support Medicare clients through wellness coaching, resource reminders, and navigation

  • Participate in periodic trainings and quality improvement initiatives (virtual)

Additional Requirements

Must maintain:


    • Valid driver’s license

    • Active auto insurance

    • Reliable transportation

  • Must pass agency onboarding and compliance checks

  • Willingness to travel up to 50% (preferred but flexible)

Position Details

Position Type: Independent Contractor (1099)
Location: Dallas/Fort Worth + surrounding areas
Hours: Flexible — contractor sets their own schedule
Compensation: Contract-based (CHW Medicare rates vary by service category)
Work Environment: Remote + field-based client visits as needed
