The Community Health Worker (CHW) will work closely with individuals and their infant(s) in the community during pregnancy, the postpartum period, and/or up to the child's third birthday. The CHW will work in tandem with a Public Health Nurse to provide personalized, community-based care to these clients. The CHW will assist clients in finding community resources which reduce their barriers to care and help ensure that the client is knowledgeable about their health and the health of their infant(s). This position is grant funded through a Community Partnership Grant.

Hiring Salary:
