We are seeking a dedicated Community Health Worker to serve as a vital link between our community and healthcare services. The ideal candidate will have experience in health education, community outreach, and supporting individuals in accessing healthcare resources while promoting wellness and disease prevention.

Role: Community Health Worker

Description:

• Manage “Unable to Reach” population: conduct telephonic outreach and home visits with members to obtain a Health Risk Screening or Health Risk Assessment.
• Expected to make 10–15 home visits per day within a 50-mile radius of home residence.
• Must have a reliable vehicle and valid vehicle insurance.
• Responsible for contacting members and community partners in assigned areas.
• Perform ongoing functions to support the Social Services department.
• Educate members about health care programs, respond to member inquiries, and support clinicians in the Physical and Behavioral Health Management department with provider and member activities.

Job Requirements:

• Bachelor’s degree OR 4 years of healthcare, social services, or community advocacy experience.
• Knowledge of medical terminology.
• Experience coordinating member-related needs, providing assistance to members, and analyzing member needs.
• Proficiency in Microsoft Office applications.
• Strong customer service skills.

Preferred Requirements:

• Community Health Worker (CHW) certification

• Bachelor’s degree in Social Work
