Position Summary
The Community Health Worker (CHW) manages a caseload of clients, providing individualized care coordination and support to remove barriers and promote healthy lifestyles, disease prevention, and chronic disease management. CHWs assist clients in achieving health goals by leveraging their strengths and connecting them to resources. Responsibilities include pre-screening, enrollment, and recertification for the AccessHealth program, conducting home visits, and accompanying clients to appointments as needed. Working under the direction of a Registered Nurse and/or Social Worker, CHWs collaborate with healthcare providers, community organizations, and support networks to ensure smooth care navigation and optimal health outcomes.
Minimum Requirements
Education
- Associate Degree or 5+ years experience in community health or related field
Experience
- 1 year experience case management or care coordination/navigation or community health in a paid or volunteer capacity
License/Registration/Certifications
- Must receive SC Community Health Worker certification within 1 year of hire
- Valid SC driver's license with a good driving record
Preferred Requirements
Preferred Education
- Bachelor's degree in community health or related field
Preferred Experience
- 3-5 years case management, care navigation, or community health experience, or related field
- Bi-lingual (Spanish) speaking preferred
Preferred License/Registration/Certifications
- SC Community Health worker certification
Core Job Responsibilities
Direct Client Services
- Manage a caseload of clients, including enrollment, recertification, and ongoing engagement.
- Conduct assessments to identify health and social needs; support clients in developing individualized care plans.
- Provide outreach and coaching to help clients overcome barriers and adopt healthy behaviors.
- Meet clients in various settings, including home visits and appointments, as needed.
Education & Support
- Educate clients and caregivers on accessing healthcare appropriately and managing health needs.
- Promote self-management skills through individualized coaching and resource navigation.
- Advocate for clients by connecting them to community resources and support networks.
Collaboration & Relationship Building
- Build and maintain positive relationships with clients, providers, community partners, and the AccessHealth team.
- Proactively collaborate with healthcare providers and community resources to support client goals.
- Identify opportunities for improvement and communicate them through coaching and feedback.
Documentation & Compliance
- Maintain accurate and timely documentation of all client interactions and care coordination activities.
- Achieve productivity and documentation standards set by the supervisor.
- Ensure compliance with organizational policies and confidentiality requirements.
