SCHEDULE: Monday-Friday 8am-4:30pm

Must be able to flex 25% of time to cover community events taking place during nontraditional work hours (Evenings and Weekends)

KEY RESPONSIBILITIES

Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. Most of the outreach to patients occurs telephonically and there are times when Community Health Specialists connect in-person with patients to deliver items and provide education. Community Health Specialists are also required to participate in team meetings/retreats, which are in-person. Community Health Specialists will be required to work in an in-office setting and must have reliable transportation to conduct community visits.

Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 25% of time).

REQUIREMENTS

High School Diploma or equivalent required.

  • 0-3 years of experience required.
  • Requires basic computer skills.
  • Requires knowledge of medical, community, and social resources.

PREFERRED QUALIFICATIONS

  • Ability to lift 20-25lbs
  • Bi-Lingual a plus.
