Registered Nurse - Community Health
Part Time - 64 hours per pay period
Includes call rotation covering evenings, weekends and holidays.
We're seeking a compassionate and highly organized Registered Nurse to join our Community Health team. In this role, you will deliver patient-centered care across hospice, home health, and public health settings while supporting community health initiatives. This position requires strong clinical judgement, excellent communication skills, and the ability to work independently in a fast-paced, community-based environment.
What You'll Do:
- · Provide comprehensive nursing care, addressing patients' physical, emotional, psychosocial, and spiritual needs
- · Assess patients through interviews, observations, and examinations
- · Develop, implement, and update individualized plans of care
- · Administer medications & treatments according to physician orders
- · Collaborate with physicians, interdisciplinary teams, and community resources
- · Educate patients and families on health conditions and care plans
- · Document patient care accurately & timely using electronic health records at the point of care
- · Maintain compliance with HIPAA, safety standards, and organizational policies
What We're Looking For:
- · Licensed as a Registered Nurse in the State of Iowa with current BCLS certification
- · Strong understanding of nursing process & community-based care
- · Excellent communication, organizational, and problem-solving skills
- · Valid driver's license, reliable transportation, and required insurance
- · Commitment to patient advocacy, quality care, and community health improvement
