Responsibilities


Job Summary:


Assist individuals and communities to adopt healthy behaviors. Conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health. May provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs, and provide services such as blood pressure screening. May collect data to help identify community health needs.


Essential Functions:


+ Advises clients or community groups on issues related to diabetes and obesity, improving general health, diet and exercise, prevention, sanitation or hygiene, self care, social issues, education, childcare or problem solving.

+ Acts as a resource to all clients, providers and the community. Attends community meetings and health fairs to understand community issues and build relationships with community members. Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community. Refers community members to needed health services.

+ Advises clients or community groups to ensure parental understanding of the importance of childhood immunizations and how to access immunization services.

+ Advocates for individual or community health needs with government agencies or health service providers.

+ Identifies or contacts members of high-risk or otherwise targeted groups, such as members of minority populations, low- income populations, or pregnant women. Identifies the particular health care needs of individuals in a community or target area. Interprets, translates, or provides cultural mediation related to health services or information for community members.

+ Provides one to one coaching to those individuals desiring this service.

+ Collaborates with Clinical Providers to provide age specific, multi-disciplinary patient/family education format that facilitates recovery, self-care, maintenance, health promotion and wellness. Consults with internal customers concerning instructional needs of patients and the community Complies with physician requests for patient instruction and coordinates instructional activities of multi-disciplinary team members.


Qualifications


Minimum:


Education and Experience:


+ HS Diploma or GED

+ Bachelors degree preferred

+ Experience working in the community

+ Teaching experience preferred


Knowledge, Skills and Training:


+ Ability to work independently, set priorities and manage work within timeframes, read and analyze technical material, communicate effectively verbally and in writing, positive mental attitude.

+ Strong customer service orientation and the ability to handle highly confidential information essential.

+ Must be computer literate.
