Position Description:

CNA Home Health Aide (Multiple Positions) for Prisendorf Care, LLC dba BrightStar Care of Plymouth, Plymouth, MN.

The Home Health Aide (HHA) provides direct patient care under the clinical supervision of a registered nurse and is under the administrative supervision of the Customer Care Manager/Scheduler. The HHA is responsible for performing various personal care services and delegated nursing tasks as necessary to meet a client's needs as ordered by the registered nurse and in accordance with the plan of care. This includes Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and certain basic clinical tasks. The HHA is also responsible for observing clients, reporting these observations to the registered nurse case manager, and documenting observations and care performed in a thorough and accurate manner. The HHA for our agency has an expanded scope of practice to include that of the Minnesota Certified Nursing Assistant (CNA). Daily travel to clients' homes within the Plymouth, MN/Western Hennepin County area to perform duties. Work schedule will be Monday-Friday, 9am-5pm and occasional weekends.

Requirements:

High School Diploma or equivalent required.

Minnesota Certified Nursing Assistant certificate required.

CPR certification to American Heart Association standard required. To include all ages, choking, and AED use.

Must be able to communicate both verbally and in writing.

Must be able to obtain by orientation:

• A negative TB screening;
• A negative urine drug test.

Must be able to pass the Minnesota Department of Human Services (DHS) background study.

Must be able to obtain a valid driver's license and proof of auto insurance per state requirements by orientation.

Must have regular access to a smart phone or tablet with cellular internet access while on duty.

Daily travel to clients' homes within the Plymouth, MN/Western Hennepin County area to perform duties. Work schedule will be Monday-Friday, 9am-5pm and occasional weekends.

Salary: $20.91-$23/hour
