The Pima County Health Department is hiring a CMHA Case Manager within our Community Mental Health, and Addiction Division. This position provides coordination of care, referrals, screenings, and helps determine eligibility for individually based, medically necessary services. This role has an opportunity for systems-level advocacy, community education, and will work closely with other community service providers, including crisis services, outpatient, housing, justice system, first responders, and various community resource agencies. Ideal candidates must display strong interpersonal communication skills, organizational skills, and be attentive to detail.

Essential Functions:

  • Conducts thorough assessments of clients' medical needs, considering physical, mental, and emotional health with additional assessment considering the social determinants of health;

  • Develops a comprehensive understanding of clients' health histories and current health status;

  • Develops individualized care plans in collaboration with clients, healthcare providers, and other stakeholders;

  • Sets health goals and strategies to address clients' medical needs and enhance overall well-being;

  • Collaborates with healthcare professionals to ensure clients receive appropriate medical care and treatment;

  • Facilitates referrals to specialists, diagnostic services, and other healthcare resources as needed;

  • Advocates for clients' healthcare needs within the healthcare system and community resources;

  • Educates clients on preventive healthcare measures, medication management, and healthy lifestyle choices;

  • Connects clients with relevant resources to address social determinants impacting their health;.

  • Maintains accurate and up-to-date records of client interactions, assessments, and care plans;

  • Compiles and analyzes data for reporting and program improvement purposes.

Minimum Qualifications:

Graduation from an accredited school of nursing AND two (2) years of professional nursing experience.

OR:

Bachelor’s degree from an accredited college or university with a major in social or behavioral science, or a closely related field as defined by the appointing authority AND two (2) years of experience providing social services.

(Relevant experience and/or education from an accredited college or university may be substituted.)

Preferred Qualifications: (Be specific in describing your experience in your application. Ensure the descriptions provided illustrate your competencies, specifically addressing the required and preferred qualifications.):  

  • Minimum two (2) years of experience working at a behavioral health or harm reduction agency conducting case management, peer support, harm reduction, or outreach. 

  • Minimum one (1) year of experience with documentation of services and reporting grant deliverables. 

  • Experience working collaboratively with community and multi-sectoral partners displaying strong partnership building skills.

  • Experience with public speaking. 

  • Bilingual (English/Spanish).

Selection Procedure:
Pima County Human Resources Department reserves the right to admit to the selection process only those candidates that meet the minimum qualifications. All applications will be assessed based on an evaluation of the listed education and experience. Candidates meeting the minimum qualifications may be further evaluated/scored against any advertised Preferred Qualifications. The hiring authority will interview and select the successful candidate from a referral list provided by Human Resources. Additional assessments/testing may be required as part of the selection process. 

Supplemental Information:

Licenses and CertificatesValid driver license is required at time of application. Valid AZ driver license is required at time of appointment. The successful applicant will be subject to a 39-month DOT Motor Vehicle Record review to determine applicant’s suitability to operate county vehicles in accordance with Pima County administrative procedures. Failure to obtain and maintain the required licenses and certifications shall be grounds for termination. Any offer of employment resulting from this recruitment is contingent upon Fleet Services’ review and approval of the candidate’s driving record. Some positions may require a current license to practice as a Registered Professional Nurse or Licensed Practical Nurse. Failure to maintain the required licensures shall be grounds for termination.

Successful candidates will receive a post-offer, pre-employment background screening to include verification of work history, education, and criminal conviction history. A prior criminal conviction will not automatically disqualify a candidate from employment with the County. Department of Labor regulation CFR Part 1910.1030 requires notification that some positions within this classification may have a high or moderate risk of blood-borne pathogens. In accordance with OSHA and Pima County guidelines Tuberculin (TB) screening, Hepatitis B vaccinations and safety training and equipment will be provided.
