We are hoping you will join us as a Clinician Sr, Behavioral Health - Clinical/Care Management - Covered California (Clinician Sr) and help shape the future of healthcare where you'll be an integral part of our Behavioral Health Integration team, helping to strive for excellence while we serve our member health with dignity, respecting the value and needs of each of our members through collaboration with our providers, community partners and local stakeholders. This position has been approved to be Full Telework.
The Clinician Sr (Behavioral Health)(Clinical/Care Management) is an advanced specialty collaborative practice responsible for providing care management services for CalOptima Health's members. You will facilitate communication and coordination among participants of the health care team and CalOptima Health's members to ensure that services are provided, which promote quality cost-effective outcomes. You will provide case management intervention on behalf of members with a behavioral health condition and may have co-occurring medical needs. Additionally, you will provide behavioral health clinical expertise, quality and cost effectiveness of proposed behavioral health services in accordance with established criteria. You will be responsible for providing back-up to the call center to handle complex and general calls, including completing telephonic behavioral health screenings and determining member behavioral health level of care needs, including telephonic behavioral health crisis intervention, higher level of care referrals and coordination of care between physical and behavioral health. You will also coordinate activities between staff, contracted providers and health networks by providing guidance and support as appropriate for CalOptima Health Medi-Cal, OneCare or Covered CA members. Together, we are building a stronger, more equitable health system.
Your Contributions To the Team:
85% - Care Management

Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
Reviews and evaluates proposed services utilizing established policies and procedures.
Analyzes all requests with the objective of monitoring utilization of services, which includes medical appropriateness and identifies potentially high-cost complex cases for high level case management intervention.
Conducts a thorough and objective assessment of the member's status for short-term cases, including psychosocial and environmental and gathers all information pertinent to the case.
Develops and implements member specific care plans and maintains appropriate documentation of patient care and progress within the care plans.
Provides frequent evaluation of members' progress, schedules follow-ups to assess progress, and if progress is static or regressive, identifies the cause and proactively initiates appropriate referrals to a higher level of case management or makes appropriate adjustments in the care plan, providers and/or services to promote better outcomes.
Establishes methods of communication and collaboration with other team members, physicians, community agencies and administrators.
Acts as an advocate in the client's best interest for necessary funding, treatment alternatives, timelines and coordination of care.
Works collaboratively with staff members from various disciplines involved in patient care with an emphasis on interpreting and problem-solving complex cases.
Documents case notes and rationale for all decisions.
Coordinates and collaborates between behavioral health providers, health networks, IPA's and CalOptima Health departments for problem resolution and service integration, including providing consultations on resolving complex case management issues and serving on the treatment team.
Builds and maintains relationships with the County Behavioral Health Plan, providers, health networks, IPA's, community-based organizations and other key stakeholders.
Provides subject matter expertise, serves as a resource and provides guidance to CalOptima Health staff.
10% - Administrative Support
Supports the CalOptima Health Behavioral Health Call Center operations.
Completes telephonic behavioral health screenings, triages for members with behavioral health needs, provides complex care coordination and takes escalated calls
Coordinates referrals when appropriate to the County Behavioral Health Plan for members who may need additional mental health and/or behavioral health case management support.
Participates in bi-weekly clinical team meetings.
Supports members in navigating the behavioral health systems of care.
Assists with training for new Behavioral Health Clinicians.
Supports the implementation of department and agency initiatives to enhance member care.
5% - Completes other projects and duties as assigned.
Do You Have What the Role Requires?
You'll Stand Out More If You Possess the Following:
2 years of behavioral health case management experience.
1 year of call center experience.
Bilingual in English and in one of CalOptima Health''s defined threshold languages (Arabic, Chinese, Farsi, Korean, Russian, Spanish, Vietnamese).

Our current hiring range is: Pay Grade: 312 - $84,092 - $134,548 ($40.43 - $64.6865).
The final salary offered will be based on education, job-related knowledge and experience, skills relevant to the role and internal equity among other factors.
