Job Summary/Objective:

The Home-Based Clinician is responsible for providing supports and clinical interventions to individuals receiving infant mental health, individual and family system interventions.  The position will span between perinatal people through 21 years of age.  Home based clinicians work in partnership with a team of home-based providers all aligned toward the improvement of a customer’s treatment goals. 

Essential Duties and Functions:

  • Implements home-based parent-infant, individual and family systems support and interventions. 
  • Completes psychosocial assessments inclusive of a diagnosis, case conceptualization, level of care assessment, and recommendations for treatment.
  • Engages in person-centered planning to develop goals and objectives. 
  • Utilizes evidenced based treatments within their scope of practice.
  • Engages in risk assessment and completes safety planning with youth and families.
  • Coordinates appropriate referrals to other youth oriented programs such as Outpatient, Wraparound, and/or Youth Peer Support Services.
  • Performs outreach prevention activities within the community.
  • May conduct educational presentations.
  • Maintains record compliance according to agency guidelines and standards as well as funding source regulations. 
  • Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements  or minimum  of  24 hours per year of clinically based trainings), implementation of evidence based practices, peer review attendance, and professional improvement practices.

 

Qualifications:

  • Master’s Degree in Psychology, Social Work or related field with Certification in the State of Michigan as a Licensed Master's Level Social Worker (LMSW) or Limited Licensed Psychologist (LLP) or Licensed Professional Counselor (LPC) preferred; Limited Licensed Master's Level Social Worker (LLMSW) or Limited Licensed Professional Counselor (LLPC) requires supervision by a fully licensed professional within their discipline.
  • Two years of experience in a behavioral health care setting utilizing evidence based practices preferred.
  • Child Mental Health Professional qualifications (as defined by the Michigan Department of Health and Human Services) is preferred.
  • Demonstrates strong verbal and written communication skills.
  • One (1) year of computer experience – Word, Excel and electronic medical record software preferred
  • Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.
  • No criminal history that would preclude employment under federal and state laws and regulations in a federal or state health program.
  • Must support a trauma informed culture of safety to aid customers in their recovery process.
