Job Description Summary:The FSP Clinician is responsible to engage members and to coordinate mental and recovery services that will ensure housing stability and community integration. The FSP Clinician is responsible for maintaining the highest quality provision of direct individual and case management services.

Job Description Essential Duties and Responsibilities:

Provide, coordinate, and implement mental health and recovery support services with individual members.Develop effective, trusting relationships with clients experiencing tri-morbid disorders, as well as the forensic population, with a focus on facilitating independence and maintenance of improved physical and mental healthCollaborate with each member to develop their individual service plan; review and update it upon completion of goalsProvide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishmentConduct screening interviews, complete housing documentation, and coordinate clients' move-in with the housing authority, property managersPerform initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housingEncourage and promote an environment that is strength based to assist clients in meeting their individual goalsIdentify appropriate permanent housing options for clients, such as, subsidized housing Section 8, Shelter Plus Care, VASH, permanent supportive housing, affordable and market rate housing, and other housing opportunitiesWork cooperatively and cohesively with other members of the FSP team, including participation in weekly staff meetings, group clinical supervision, and trainingsDevelop and maintain a complete, accurate, and current member file with all funder-required documents

Job Description Qualifications:

MSW or MFT who is BBS eligible (supervision hours are available for this position)Must be registered with the board of behavioral sciencesAt least one year experience working with clients with mental illness, chronic health issues, and substance use disorders.Demonstrated knowledge of clinical case managementKnowledge of community resourcesAble to work in a high-tolerance, harm reduction model with individuals who have multiple barriersStrong interpersonal and team building skillsComputer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs.Current, valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.Able to obtain and maintain CPR/First Aid certification

Job Description Preferred Qualifications:

Minimum 30 WPM typing speedBilingual Spanish preferredExperience working with homeless individuals who are living with mental illness, substance addiction physical health issues, and forensic population preferred
