Job description:

The Therapist serves as a direct clinical provider responsible for delivering high-quality behavioral health services, including individual, group, and family therapy. This role works closely with a multidisciplinary treatment team to support clients experiencing mental health and substance use challenges.

Under the supervision of the Clinical Director, the Therapist manages an assigned caseload and provides comprehensive clinical services including assessments, treatment planning, and discharge planning.

Qualifications

  • Master’s Degree in a behavioral health discipline
  • Current or license-eligible in Indiana (LCSW, LMHC, LMFT, PsyD, LPCC)
  • Minimum 2 years of experience in a behavioral healthcare setting
  • Residential treatment experience preferred
  • Knowledge of behavioral health and addiction treatment best practices
  • Familiarity with Medication-Assisted Treatment (MAT) services preferred
  • Strong clinical judgment and decision-making skills
  • Ability to work collaboratively within a multidisciplinary treatment team
  • Strong communication and interpersonal skills
  • Proficiency with electronic medical record (EMR) systems
  • Commitment to maintaining HIPAA compliance and client confidentiality
  • Professional demeanor and ethical practice

Key Responsibilities

  • Provide individual, group, and family therapy to clients
  • Complete biopsychosocial assessments and admission documentation
  • Develop and maintain individualized treatment plans
  • Assign appropriate diagnoses and update treatment plans regularly
  • Maintain accurate and timely documentation in the EMR
  • Facilitate group therapy sessions and complete related documentation
  • Participate in multidisciplinary treatment team meetings
  • Collaborate on discharge planning and aftercare coordination
  • Conduct suicide risk assessments and document findings
  • Maintain compliance with HIPAA and organizational policies
  • Participate in performance improvement activities
  • Maintain current licensure and required certifications
  • Follow facility policies regarding client safety, infection control, and PPE usage
  • Report safety concerns to leadership when necessary

Clinical Documentation Responsibilities

  • Contact assigned clients within 24 hours of admission
  • Complete Biopsychosocial Assessment, Admission Note, ASAM Level 3.5 Criteria, SUD Treatment Plan, and Diagnosis within 24 hours
  • Conduct weekly 1:1 therapy sessions
  • Update treatment plans and ASAM criteria every seven days
  • Complete group session documentation in the EMR
  • Review case management progress notes weekly
  • Complete discharge documentation, including:
  • Columbia Suicide Severity Scale
  • ASAM Discharge/Transfer Criteria
  • Discharge Summary and Aftercare Plan

  • Maintain strict client confidentiality
  • Follow policies regarding AMA (Against Medical Advice) discharges
  • Demonstrate professionalism, ethical practice, and cultural sensitivity
  • Maintain a strong commitment to client, family, and referral partner satisfaction
