Summary: The NP/PA Clinical Support Team Provider will serve as an integral member of the Petaluma Health Center's primary care team and offer critical treatment plan follow up and support for patients at Petaluma Health Center's sites. The Clinical Support Team Provider will ensure that patients, particularly those at high risk or with complex care needs, have access to timely and appropriate care between visits with their primary care providers. Critical to the role will be the ability to navigate the electronic health record (EHR) and manage the flow of information to improve health outcomes for patients.

Salary Scale: $120,000 to $173,806 per year
The compensation amounts listed above do not include relocation assistance, sign-on bonus, productivity or quality/service incentives, call pay, extra shift incentives, if applicable to the position.
