As our teams continue to grow, we're seeking a therapy leader who understands the complexity of home care and wants to help clinicians succeed, navigate challenges, and elevate care across the organization.
You won't sit behind a desk simply reviewing charts.
You will be helping field clinicians think through difficult situations, serving as a sounding board, providing guidance on complex cases, and creating an environment where staff feel supported and empowered.

Clinical Supervisor PT/OT Overview:
• Become a trusted clinical resource and leadership partner for therapists, nurses, and field clinicians throughout the organization.
• Support clinicians navigating difficult patient situations, complex family dynamics, transitions of care, and clinical decision-making.
• Serve as a mentor and coach by helping clinicians build confidence, sharpen skills, and continue growing professionally.
• Foster a culture where clinicians feel supported, heard, and empowered to provide exceptional patient care.
• Conduct regular chart audits, case reviews, and quality initiatives while identifying opportunities to elevate care delivery.
• Collaborate with intake, scheduling, and leadership teams to create seamless patient experiences.
• Assist field staff with problem-solving, escalation concerns, and navigating challenging scenarios.
• Support onboarding, orientation, and ongoing staff development initiatives.
• Lead quality and process improvement efforts focused on patient outcomes, satisfaction, and reducing hospitalizations.
• Participate in on-call rotation and provide leadership support to clinicians when they need guidance most.
Clinical Supervisor PT/OT Qualifications:
• Active unencumbered Minnesota PT or OT license required.
• Prior Home Health Care experience required.
• Strong knowledge of Medicare guidelines, OASIS, and home health regulations.
• Leadership, supervisory, mentoring, preceptor, or clinical coaching experience strongly preferred.
• Strong communication skills and ability to guide difficult conversations with confidence and empathy.
• Ability to serve as a calm and trusted resource during challenging situations.
• Experience with EMR systems and electronic clinical documentation.
• Strong critical thinking and clinical decision-making abilities.
• Ability to thrive within a fast-paced, collaborative environment.
