Job Responsibilities:
Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
Collaborate with multidisciplinary team members to optimize patient care and outcomes.
Educate patients and their families on health maintenance and disease prevention.
Ensure accurate documentation and compliance with all regulatory guidelines.
Case Management for other CNAs, CHHAs.
Requirements:
Active RN license in the state of practice.
Previous experience in a home care or acute care setting.
This is a full-time, in-person role, requiring presence in the office 3 days per week.
Responsible for administrative duties, field RN case's, oversight, assessments, and coordination of care.
Balance office responsibilities with field support while maintaining leadership accountability.
Exceptional interpersonal and communication skills.

Compensation: $90,000.00 - $120,000.00 per year
