Job Summary:

The Clinical Social Worker/Patient Advocate provides services for the patients and families of the Program in Women’s Oncology to address psycho-social, environmental, and behavioral factors which have impact on the healthy functioning and/or recovery of the patients and families. The incumbent functions as a member of the health care team to provide direct clinical services including assessment and coordination of social services. The incumbent develops a network of community agencies to facilitate referrals. Performs other duties as requested. The incumbent will plan and facilitate support groups throughout the program.

Duties & Responsibilities:
  1. Demonstrates competency, individually and as part of the multidisciplinary team, in performing clinical care and assessment of all patient populations.
  2. Provides clinical social work services to patients/families/significant other, which include assessment and referrals.
  3. Collaborates and interacts with members of health care team and other hospital staff to coordinate services.
  4. Maintains required patient records, including collaboration with fiscal on billing.
  5. Develops and maintains network of community agencies and facilities to expand resources, facilitate patient access, and provide services to patients and families.
  6. Plans and facilitates support groups across the program.
  7. Participates in development and implementation of programs, policies and procedures.
  8. Preserves and protects volunteers and patients' rights to confidentiality.
  9. Performs other job-related duties as required or requested.
Requirements:
  • Master’s degree in social work and a license as a Clinical Social Worker (CSW) or if hired prior to 5/2/2011 a Licensed Mental Health Worker (LMHW) required and a license as a Licensed Independent Clinical Social Worker (LICSW) preferred.
  • Must have first-hand knowledge and/or experience with Oncology patients in the cancer treatment setting.
  • Excellent interpersonal and communication skills required. 
  • Supervisory and administrative experience helpful.
  • Group facilitation experience is preferred
