Review medication lists, vitals and labs to determine appropriateness for clinical pharmacy program. Patient outreach to schedule with Clinical Pharmacist and Primary Care Provider (PCP) for medication therapy and disease management. **\$17.00 - \$22.25 DOE.** **MINIMUM QUALIFICATIONS:** High school diploma/GED. Idaho Board of Pharmacy Technician certification preferred and in good standing. 3-5 years experience in a retail pharmacy or as clinical staff in a medical clinic preferred but not required. Must be fluent in Spanish, preferably certified as a medical interpreter. **KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:** - Computer skills include the ability to use Pharmacy Management software, NexGen (EPM & EHR), and Microsoft software products, and the ability to quickly learn new applications. - Effectively communicate verbally and through written (electronic) correspondence between members of the pharmacy and clinical team - Basic knowledge of drugs and their indication - Basic knowledge of vitals, labs and other objective information obtained - Electronic organization of records and spreadsheets - Bilingual Spanish skills - Medical/behavioral health terminology - Ability to ensure privacy - Strong organizational skills - Ability to work on multiple projects simultaneously - Work effectively with a wide range of responsibilities - Detail oriented - Always maintain professional conduct and appearance - Establish and maintain effective working relationships with providers, staff and management **DESCRIPTION OF DUTIES FOR CLINICAL PHARMACY TECHNICIAN:** 1. Azara Reports 1. Run and review timely reports to identify patients for clinical pharmacy services i\. Hypertension Controlling High Blood Pressure ii\. Diabetes A1c or GMI \>9 or untested iii\. Statin Therapy for the prevention and treatment of CVD iv\. Aspirin Use v\. Other reports requested by clinical pharmacist 1\. Including but not limited to insurance medication adherence, statin use in diabetes, concurrent opioid and benzodiazepine prescribing, poly-anticholinergic reports Organize reports and update them according to patient outreach that occurred 2\. Documentation/Tracking: a\. Keep spreadsheets organized with appropriate outreach b\. Review patients charts, including medications, vitals, labs, or other objective data to help with appropriate patient identification for clinical pharmacy services c\. Responsible for accurate, timely, and complete documentation in patient records within EHR d\. Update reports and track patients appropriately for scheduling and follow-up purposes e\. Track and update Goal spreadsheet to track number of phone calls and appointments made. 3\. Outreach and communications: 1. Contact patients and discuss that clinical pharmacy services would benefit the patient, expectations regarding the visit and get them scheduled with PCP and clinical pharmacist at assigned clinics at least 4 weeks from last appointment with PCP. 4.Accompany provider into the visit with the patient and accurately interpret the session between the patient and provider. 5\. Perform any ancillary duties required by the provider during and after the visit. 6\. When not working as an interpreter, will perform duties as assigned which could include but are not limited to: a\. Work on reports b\. Assist in the clinic with miscellaneous tasks and processes. 1. Other responsibilities: 1. Run test claims through Pharmacy software at the request of clinical pharmacists to help determine coverage of devices and medications 2. Review and obtain patient assistance paperwork for patients using clinical pharmacy services 3. Complete tasks as assigned by clinical pharmacists 4.
