Reporting to the Clinical Pharmacy Manager, this position is responsible for the operational oversight, clinical
development, and management of adult ambulatory clinic pharmacist programs. Responsibilities include, but are not limited to, employee supervision; education and training of pharmacists; technicians, patient advocate specialists; the analysis, development and growth of clinical pharmacy services including, but not limited to, implementation of CDTMs and credentialing and privileging in the adult ambulatory clinics; management of business activities related to pharmacist medication management and revenue capture from medication orders; quality assessment and process improvement; coordination of pharmacy services and collaboration with other hospital departments; teaching of pharmacy interns and residents including directly or indirectly overseeing the PGY2 Ambulatory Care Pharmacy residency program; maintenance of practice standards, patient services and compliance with accreditation standards and applicable laws and regulations.
This position is responsible for the provision of specialized clinical services to patients as part of the OHSU Specialty Pharmacy Program. The Clinical Pharmacy Supervisor will be responsible for the development and implementation of additional Specialty Pharmacy Program opportunities and will be expected to contribute to the financial viability of the program, department and organization.
Additionally, as a licensed pharmacist, the position will provision specialized clinical and/or comprehensive medication management services to patients in the ambulatory setting to adult patients.
This position will function at higher decision-making levels regarding drug therapy, proactively affecting drug use by making recommendations at the time prescribing decisions are being made or, in some cases, making independent decisions about drug therapy in cooperation with other health team members. This position is responsible for the direct clinical supervision of patients, and ensuring compliance with all legal, regulatory and institutional directives. This supervisory pharmacist position will interact directly with patients, prescribers, nurses, and care coordinators to develop an interdisciplinary approach to patient care.
Function/Duties of Position:Monitors medication therapy for response and adverse reactions. Collaborates with providers to ensure safe and appropriate therapy through the use of a Collaborative Drug Therapy Management (CDTM) agreement. Appropriately reports all adverse drug reactions, drug defects, and medication errors. Conducts prospective and retrospective drug utilization reviews to ensure optimal therapy.
Counsels and trains patients regarding the appropriate use of their medications (i.e., purpose, actions, dosage, side effects, storage, precautions etc.).
Participates in drug product selection, advises providers regarding the selection of non-formulary drugs while offering therapeutic, cost effective alternatives.
The pharmacist will experience competing demands on their time from directors reports, clinic staff and/or
patients and will need to effectively multi-task and prioritize demands. The pharmacist must deal with intense
situations on a daily basis. The expectation will be to provide clients with excellent customer service.
Additionally, within the healthcare setting there can be a significant lack of control over the work pace, with
frequent interruptions (work is often dictated by external factors) that may lead to mental fatigue or stress.
