Responsibilities:Provides pharmaceutical care services, training and support to the staff and physicians of UF Health Central Florida focused glycemic management and HCHAP Medication Scores. Is responsible for glycemic management including the optimal use of hyperglycemic medications and improving medication-related HCHAPS scores. Is involved in various activities that include, but not limited to Participation on Quality Improvement Teams, Focus on Core Quality Measures, Drug Use Evaluations. Attendance and involvement in varied medical staff committees/meetings; adverse drug reaction reporting and follow-up, formulary maintenance, one on one physician engagement. The focus of this position will be to train and develop pharmacists and physicians to provide evidenced care directly to patients.
