The Epilepsy Care Program creates a unified, accessible entry point for comprehensive seizure care, prioritizing consistent, equitable, and patient-centered experiences through an interdisciplinary team that includes Neurology, Advanced Practice Providers, Social Work, and Pharmacy. Pharmacists play a critical role in navigating complex antiseizure medication selection, monitoring drug interactions, and addressing access barriers, while social workers provide essential support in care coordination, psychosocial needs, and resource navigation. This integrated approach ensures that patients receive continuous, adaptable care that reflects both clinical complexity and real-world needs over time.
